This permit is required for all mobile food vendors operating under the requirements of the City of Puyallup Mobile Food Vendor Pilot Program, in effect until October 31, 2020.
You will be required to attach proof of Health Department Approval, Insurance (with CG 20 12), and Hood Suppression Inspection Tags.
